Chest heaviness and palpitations only when lying down

Hello, I hope you can help. When I lie down at night, I feel an increase in what I would describe as chest pressure. I am aware of palpitations and my heart bounding and irregular. During the day I often have periods of dizziness I am on Inderal for migraine thrapy and have been on a 40 mg dose x 2 daily for two years now. I was told I have an MVP ten years ago but no one seems to hear it or see it on repeat echo. Despite the dose of Inderal, my last two appointments with my PCP revealed high blood pressure

Should I be worried about the pressure and irregular beats?
